Brief Description:
This is a request for the renewal of the Waiver to provide home and community-based supports to eligible adults with developmental disabilities. The supports provided are designed to prevent or delay out-of-home residential services for participants or to provide residential services in the least restrictive community setting for participants who would otherwise need ICF/MR level of care.
