The Arc Cross-Disability Database Legislation is now being implemented. This is not just another “needs survey” that is going to lie on the shelf. It is a mandate of the General Assembly that has been signed into law by Governor Blagojevich, which makes it much different from any other needs survey you have completed in the past. Whether you need services now or in the future, it is important that you make your needs known now. These services may include residential, vocational, in-home supports, etc. The Illinois Prioritization of Urgency of Need for Services is being administered by the local Individual Service Coordination agency (ISC/PAS) in your area. There are 18 ISC agencies around the state. If you do not know the location of your ISC agency, click Illinois Life Span and then select your county and use the Search Database pull down menu and select ISC. Then click Search and select any program listed to find the location of your ISC. For a copy of the Illinois Prioritization of Urgency of Need for Services form, click on the link below. To be entered into the database, you will have to work through your ISC agency, but we have included the application here for your information. You want your needs known, so it is in your best interests to be part of this state-wide database. This is really important to you.
