I would like to recommend that all CILA providers cooperate in this mandated survey that
you will be getting in the near future. Please note that the survey is not included in this
notice but it will be forth coming.

I wanted to give you a heads up regarding a survey that will be distributed to all CILA
providers very soon. As you already know, in March, federal CMS adopted new rules
related to “community settings” for HCBS waivers. Currently HFS is working with the
Departments on Aging and Human Services to develop a required statewide transition
plan for CMS’ review. This plan will include input from stakeholders and we look forward
to your involvement. As part of the development of the statewide transition plan, HFS
and the Departments on Aging and Human Services have partnered with the University
of Illinois at Springfield to create a survey for residential HCBS programs, which includes
CILA’s. The goal of the survey is to assess the waivers’ current compliance with the new
community setting regulations.
The cover letter will be mailed to providers yet this week notifying them of the survey.
One portion of the survey is agency/provider specific and the other is setting specific,
or in other words, specific to an individual building. In cases where multiple CILA’s are
owned or operated by a particular provider, a designated corporate staff person will
receive the surveys instead of the individual CILA. In this instance, only one agency/
provider survey needs to be completed and then separate settings surveys will be
submitted for each CILA. There are no right or wrong answers on the survey.
Additionally, this is not a review of compliance with CILA rules. Again, the survey will
help HFS determine what needs to be included in the statewide transition plan to show
current and future compliance with the new federal rule requirements.
Because the information obtained from this survey is critical to the development of the
statewide transition plan, it is mandatory. I do not anticipate any hesitation from CILA
providers to cooperate. I will also be sending an e-mail to each CILA or corporate staff
person (as applicable) letting them know to expect this survey. I hope you will
