To: All Home and Community-Based Services (HCBS) Waiver Providers and Nursing Home Facilities

Re: Integrated Care Program Service Package 2

This notice is to inform Home and Community-Based Services (HCBS) waiver providers and Nursing Facilities of the implementation of Integrated Care Program (ICP) Service Package 2.

ICP is a program for seniors and persons with disabilities who are eligible for Medicaid, but not eligible for Medicare. The program is mandatory and operates in suburban Cook (all zip codes that do not begin with 606), DuPage, Kane, Kankakee, Lake, and Will Counties. Two Managed Care Organizations (MCOs) – Aetna Better Health and IlliniCare Health Plan – manage the care of the approximately 36,000 ICP enrollees.

ICP is being implemented in three Service Packages. Service Package 1 began on May 1, 2011 and includes all standard Medicaid medical services. Effective February 1, 2013, Service Package 2 will be implemented to expand ICP coverage to include Long Term Supports and Services (LTSS), including those in nursing facilities or those who receive Home and Community-Based Services (HCBS) waivers. Waiver services through the Home Services Program (HSP), the Community Care Program (CCP) and the Supportive Living Program (SLP) will now be covered by the MCOs. Service Package 3, which will include the waiver services for Persons with Developmental Disabilities, will be implemented at a future date.

There are 2,957 HCBS waiver enrollees and 2,601 nursing home residents who will have their Service Package 2 waiver services transitioned to Aetna Better Health or IlliniCare Health Plan on February 1, 2013.

If you are currently a provider serving the ICP population, HFS encourages you to work with Aetna Better Health and IlliniCare Health Plan so clients may continue to receive health care services through your organization. HFS believes that it is in the best interest of clients for their providers to remain the same.

Being a provider with ICP means:

  • You are providing continuity of care for the most at-risk Medicaid clients; and
  • MCOs payments may come sooner than HFS payments.

As of February 1, 2013, the only way to ensure payment for clients enrolled in ICP is to contract with the health plans. If you have not already done so, we encourage you to contact them as soon as possible to begin the process.
