HFS staff informed us today (Monday) that they submitted to CMS the revised edition of the Home & Community Based Services Statewide Transition Plan.
They have requested that the HFS website be updated with a link for the public to the revised plan. In the meantime, they shared with us the Statewide Transition Plan.
Thank you to those who provided comments to HFS on the earlier draft. I am sure we will be in touch as the State continues to work with CMS on final acceptance of the plan and its implementation.
The State is currently working to finalize the site visit validation tool—which was previously shared for comment—in order to begin the site visits yet this fiscal year.
