Advocates will once again want to provide written comments on the revised Home & Community Based Statewide Transition Plan.

A little background, CMS has published new rules governing home and community based services/supports. They are a long time in coming and supported by The Arc. The new rules stress person-centered planning, employment and community inclusion. The first Illinois Transition Plan was sent back to the state, hence the revised transition plan.

My first quick look at the revised plan is that it is not much different from the first plan but dates have changed. I need to spend some time going through this and will post comments for your use.

Next Thursday, Director Fenton is hosting a meeting of the State Associations to discuss the revised plan so that should serve as a good starting point.

The public comment period for the revised HCBS Statewide Transition Plan will begin on December 4, 2015 and end on January 3, 2016.

On the 4th, this Friday, the HCBS Transition Plan webpage, maintained by HFS,  located at http://www.illinois.gov/hfs/MedicalClients/HCBS/Transition/Pages/default.aspx

The webpage will be updated to include a link to the revised Plan, public notice and instructions on how to submit public comments.
