The Dept. of Human Services has posted the Request for Information/Request for Application information on their website. I encourage all service providers to enroll and show your interest in rebalancing the disability system. As we discussed at the last Executive Forum meeting, there is no downside to filling out the applications because you will have the opportunity to discuss this process before formal contracts are agreed upon.
Go to www.dhs.state.il.us/page.aspx. The RFI/RFA form and instructions can be found by typing in RFI in the search box. Number 9 is the form, and number 10 is the instructions.
