Per Marty Ford, The Arc of The United States, we have received a request from
Senator Tom Harkin (D-IA), Chairman of the Senate Health, Education, Labor
and Pensions (HELP) Committee.  We would like your assistance in responding
to his request.

The HELP Committee is conducting a questionnaire on the topic of disability and
poverty. The subject of poverty and disability represents a topic of major interest
to Chairman Harkin. He would like to hear directly from people with disabilities
living throughout the U.S. to learn more about their perspectives on this issue.
Chairman Harkin’s plan is to publish a report on the issue and to possibly hold a
hearing before he leaves the Senate next January.

The HELP Committee would like assistance from The Arc to contact individuals
with disabilities to complete the questionnaire on disability and poverty. We would
like for each of you to contact one individual with disabilities to fill out the
questionnaire by Friday, July 25, 2014. The questionnaire would be completed by
the individual with disabilities and returned by email directly to Senator Harkin’s
staff, Michael Gamel-McCormick, at Michael_gamel_mccormick@help.senate.gov.
Also, please ask the individual to mention, in the cover email, that The Arc referred
him/her to complete the questionnaire.

Please let me know if you have any questions (ford@thearc.org).

Thank you!
