DESCRIPTION:Presenter: Claire Cook\, DSCC and Emma Nelson \nA successful transition to adult healthcare is an important part of preparing youth and young adults with special healthcare needs and disabilities for life after high school. Yet healthcare transition is often overlooked in the Transition IEP. Using real-life examples\, this session will explore how healthcare transition goals can be included in the IEP\, ways youth and families can prepare for adult healthcare\, and strategies to help build confidence for the future. \nAt the end of this session\, participants will be able to: \n1. Explain why healthcare transition is an important part of preparing youth with special healthcare needs and disabilities for adulthood. \n2. Identify opportunities to incorporate healthcare transition goals and activities into the Transition IEP. \n3. Apply strategies and resources that help youth build confidence and self-management skills for adult healthcare. \nClaire Cook is the Title V Transition Specialist at UIC: Division of Specialized Care for Children (DSCC). Claire’s role at DSCC is in helping youth with special healthcare needs make a smooth and successful transition to all aspects of adult life. \nEmma Nelson is a skilled trade student from Southern Illinois working toward her goal of becoming a Nuclear Medicine Specialist. Her interest in healthcare is deeply rooted in lived experience. Growing up in a medically complex family\, Emma helped support her younger siblings through significant medical challenges\, including frequent hospitalizations and multiple high-risk surgeries for her youngest sibling. At the same time\, Emma has spent the past five years navigating and advocating for her own care while living with chronic pain and connective tissue disorders.
