Hope is at the heart of the Family Transition Project. We encourage people with intellectual/developmental disabilities (I/DD) living in large institutions (and their families) to explore the possibilities of community living. We share our own success stories in making the transition to our communities of choice and the successes of our loved ones and those of many other FTP families.

We provide compassionate support to individuals and their families, advocacy, helpful information and connections, and our own lifelong experience with the challenges of transitions and the benefits of full inclusion.

Family advocates meet one on one with self-advocates and their families by phone, email, or in person – whatever is most helpful. We are here to listen and learn their histories, their needs, and their concerns.

We will continue to serve as a resource of friendly support for individuals and families as they iron our transition issues, even after community living is achieved.
